This form must be completed by the parent or carer of the child who will benefit from the laptop. Applications from those who are not the parent or carer will not be accepted. Please read the Tech4Families scheme Terms and Conditions. Download a PDF version of this form.

Once you have applied using the form below, we will request your verifier completes a form confirming your details and need for a donated device. Your verifier has 14 days to complete this form, otherwise your application will be dismissed. If verified successfully, we will notify you and review your application. Please note: If your application is approved, it can take two to three months before your laptop will arrive.
